Your Guide Through the Courts
The Self-Service Center is your source for help navigating the court system in Palm Beach County, providing the resources you need to represent yourself and much more.
Self-Service Centers are located in the Main Courthouse (room 1.2500 on the first floor) and in all branch courthouse locations. We are open Monday through Friday, 8 a.m. to 4 p.m.
Notary, copy and fax services are available, as well as public access computers.
Legal Forms
- Free forms: Download free PDFs of many forms.
- Order form packets: Include step-by-step instructions; may be purchased at our Self-Service Center locations, by calling (561) 355-7048 or online. Online orders will be sent by email as fillable PDF documents.
Packets will be delivered within one business day of receipt of payment.

Once you have completed your forms, you have the option to file them electronically, by mail or in person.
It is important to include your name, address and telephone number on every document you file with the court.
Navigator Document Preparation Assistance
Free document preparation assistance is available by appointment at our West Palm Beach, Palm Beach Gardens and Delray Beach locations. The Navigator can assist you in completing court documents and can give you procedural information for most family law and county civil actions. They cannot give you legal advice or represent you in court.

Email Designation
You must provide an email address for service of court documents unless you are in custody or have been excused because you do not have an email address or do not have regular access to the Internet. Use the forms below to update your email address or request to be excused from email service.

Notice of Confidential Information Within Court Filing
All filers of circuit civil, county civil and small claims court documents are solely responsible for ensuring that confidential information is redacted or is identified for redaction.
Any court filings made in a Florida court that contain confidential information that has not been redacted must be accompanied by a Notice of Confidential Information – a form that lists exemptions to public records laws, including adoption records, Social Security or bank account numbers, HIV test results or information identifying victims of sexual offenses.

Designate E-Filed Case-Initiating Documents That Contain Confidential Information
If you use the Florida Courts E-Filing Portal to file in a Circuit Civil (CA), County Civil (CC) or Small Claims (SC) case, you must designate if case-initiating documents contain confidential information. Filers will be prompted to select one of three options on the Documents page to denote the filing as containing no confidential information or is accompanied by a Notice of Confidential Information or Motion to Determine Confidentiality.
